Houston Community Surgical Insurance Options

Insurance & Pricing

Dr. Belizaire has designed her practice to give patients transparent pricing, no surprise bills, and faster access to care.

As such, Houston Community Surgical does not have contracts with any commercial insurance plans. However, you can still use your insurance benefits for the majority of surgical procedures, often with the same or lower out-of-pocket costs as in-network care.

In-Network Plans

Traditional Medicare
Tricare

PPO Commercial Insurance with out-of-network (OON) benefits

Can be used like “in-network” benefits for surgery. This includes both routine and specialty care (see lists below).
Your out-of-pocket costs are the same as or less than seeing an in-network surgeon and you will never receive a surprise bill from Dr. Belizaire’s office.

HMO/EPO Commercial Insurance, PPO without out-of-network benefits

Can be used like “in-network” benefits for specialty surgical care only.
Routine care is generally not covered by insurance, but patients can elect to pay out of pocket for these services if desired.

Specialized Surgical Care

Pelvic floor disorders – rectal prolapse, rectocele, fecal incontinence
Proctologic (anorectal) conditions – hemorrhoids, fissures, fistulas, condyloma
Colorectal cancer, Anal cancer
Endometriosis
Diverticulitis
Inflammatory bowel disease – Crohn’s and Ulcerative Colitis
Ostomy-related issues
Pilonidal disease
Hidradenitis suppurativa
Emergency surgery

Routine Surgical Care

Elective gallbladder disease
Umbilical or inguinal hernias
Screening colonoscopy
Benign soft tissue lumps and bumps

In-Office Visits & Procedures

All new-patient consultations: $50
All in-office procedures: Cash-pay, unless covered by Traditional Medicare or Tricare

Why This Practice Model?

After 2 years of contracting with the insurance companies, Dr. Belizaire intentionally elected to cancel commercial insurance contracts to avoid the surprise billing that often happens with in-network surgical care. She found that patients were paying premiums to have their health care covered yet insurance companies routinely denied care. As an out of network surgeon, Dr. Belizaire can provide clear up-front pricing for her services, more time with each patient, faster access to surgery, and personalized, patient-centered care.
